  • Skilled nursing care: Registered nurses can visit your loved one's home to perform health assessments, monitor medications, and provide wound care.

  • Care management: A dedicated care manager will work with you and your loved one to create a personalized care plan, coordinate services, and meet all their needs.

Eligibility for the Massachusetts HCP and ECOP programs

Only some qualify for these programs. To be eligible for these home care programs, you must meet specific criteria:

Age and residency requirements

  • Be at least 60 years old OR have early-onset Alzheimer's/dementia
  • Live at home or with a caregiver, not in a facility
  • Be a Massachusetts resident

Health and disability requirements

  • Need aid with daily tasks as determined by an assessment
  • For ECOP, meet the higher clinical level required for nursing home admission

What financial requirements are needed?

  • For a single person in 2023, have an annual income below $33,659
  • For a couple in 2023 have a combined annual income below $47,626
  • Income level determines if co-payments for services are required

How do family caregivers get paid under these programs?

Under the consumer direction option, family members (except legal guardians or surrogates) can be hired and paid as caregivers to provide approved services for their loved ones participating in HCP or ECOP. Family caregivers must meet any training or credential requirements.
