The Community Care Services Program (CCSP) is a Medicaid home and community-based waiver services program in Georgia that provides a lifeline to vulnerable individuals by offering community-based social, health, and support services as a viable alternative to nursing home placement. Administered statewide by Georgia's Division of Medicaid in partnership with 12 Area Agencies on Aging (AAAs), this program is a testament to the transformative power of compassionate caregiving.

Overview of the Community Care Services Program

The primary purpose of the Community Care Services Program is to enhance the quality of life for eligible consumers by providing a wide array of community-based social, health, and support services.

The program's target population includes elderly and disabled individuals who meet the Georgia Department of Community Health's (DCH) Division of Medicaid eligibility criteria. CCSP aims to address these vulnerable individuals' unique needs and challenges, enabling them to age with dignity, maintain independence, and foster a sense of belonging within their local communities.

CCSP's goals revolve around promoting individual choice, self-determination, and community integration. By empowering eligible consumers with the ability to make decisions about their care, CCSP strives to ensure that each participant's preferences and values are respected. Through a person-centered approach, the program seeks to foster a supportive environment that encourages independent living while providing essential services tailored to meet the diverse needs of its beneficiaries. Additionally, CCSP aims to alleviate the strain on nursing homes and reduce institutional care costs by offering a cost-effective alternative that prioritizes the individual's well-being.

Eligibility Requirements for Community Care Services Program

To qualify for the CCSP, individuals must meet specific eligibility criteria prioritizing their need for home and community-based services over nursing home placement. The following are the detailed eligibility requirements for CCSP services:

  1. Functional Impairment: Eligible individuals must have functional limitations due to physical conditions. These impairments may hinder their ability to perform everyday activities independently and require assistance with daily living tasks.
  2. Physician Approval: A physician must approve the need for an intermediate level of nursing home care and work with the individual to create a care plan that addresses their needs and preferences.
  3. Medicaid Eligibility: Participants must be eligible or potentially eligible for Medicaid based on a financial screening. Medicaid eligibility is essential to access the funding necessary to support the services provided by CCSP.
  4. Medicaid Eligibility after Admission: Following admission to CCSP, the individual must maintain Medicaid eligibility to continue receiving program benefits.
  5. Choice of Home and Community Services: Eligible individuals must actively choose to receive services in their homes and communities, expressing their preference for this care over nursing home placement.
  6. One Waiver Program at a Time: Participants can only be enrolled in one waiver program at a time to ensure efficient and coordinated service delivery.
Note: CCSP recognizes Alzheimer's and other types of dementia as physical conditions that may qualify individuals for the program based on their functional impairments.

These services are tailored to meet each participant's unique needs and preferences and are coordinated by a dedicated care coordinator. The following is an overview of the available services:

  1. Adult Day Health: Participants can access medically supervised group day programs at adult day health centers, including physical, occupational, or speech therapy providing individuals with a supportive and engaging environment while offering necessary therapeutic interventions.
  2. Alternative Living Services: State-licensed personal care homes provide 24-hour personal care supervision for individuals who require more extensive support and supervision in a residential setting.
  3. Emergency Response Services: In-house, 24-hour electronic two-way communication systems are provided to call for help in emergencies, ensuring the safety and security of participants.
  4. Home-Delivered Meals: Eligible individuals receive nutritionally balanced meals delivered directly to their homes, promoting their overall health and well-being.
  5. Home-Delivered Services: Skilled nursing and therapy services are provided in the participant's home to continue their home health benefit, offering vital medical support.
  6. Out-of-Home Respite Care: Caregivers can benefit from temporary relief through out-of-home respite care, allowing them time off. At the same time, the participant receives care in a personal care home or adult day health center.
  7. Personal Support Services: In-home services like personal care, meal prep, light housekeeping, and shopping are available, offering valuable assistance to individuals in the comfort of their own homes. Additionally, in-home respite services support caregivers who need periodic breaks from their caregiving responsibilities.
  8. Structured Family Caregiver: Eligible caregivers who live with and assist a waiver participant requiring substantial assistance with activities of daily living may get additional support through the Structured Family Caregiver Program.

Compensation for family caregivers

The Community Care Services Program (CCSP) compensates family caregivers through the Home and Community-Based Services (HCBS) Waiver. The HCBS Waiver provides financial support for eligible family caregivers caring for their loved ones. This compensation is based on the care recipient's level of need and the approved number of hours for caregiving.

The Structured Family Caregiver program is a separate initiative that may offer additional support to eligible family caregivers who live with and assist an elderly and/or disabled waiver participant requiring significant assistance with activities of daily living. This program may provide further help and resources to qualifying family caregivers, but it is not the primary means of compensation for caregivers under CCSP.
